Requirements for Senior Highschool Graduates

• Accomplished Application Form
• 2 Photocopies of Birth Certificate (Original must be presented upon visit)
• 2 Photocopies of Baptismal Certificate (Original must be presented upon visit)
• 2 pieces 1 x 1 ID Picture with white background
• Pass the preliminary interview
• Pass the Mams Entrance Exam with satisfactory rating (Entrance exam fee: Php 200.00)
• Original copy of report card (Form 138)
• Good moral certificate from the previous school
• Medical exam results (Urinalysis, Fecalysis, CBC)
• Drug test result (Any diagnostic center)
• Report together with the parent / guardian for final interview.
